This form is used to request approval for the development or revision of online courses at Cairn University. The request is not official until signed by the Provost. Once signed by all parties, the form serves as the official contract for the course development. Please refer to the Distance Education Handbook for more details regarding the Cairn course development process.
SECTION I: COURSE DEVELOPMENT INFORMATION - Completed by the Dean

SECTION II: FACULTY INFORMATION/DEADLINES - Completed by the Faculty Developing the Course
All course content must be submitted to the Office of Distance Education in three stages and no less than 30 days prior to the start of the course. Start by choosing the third deadline and then select deadlines 1 & 2. The faculty is expected to meet the selected deadlines.

- ii. Standard Process: Other Programs
SECTION III: OFFICIAL COURSE DEVELOPMENT APPROVAL - Completed by the Provost
The Provost approves the course development including the compensation amount, terms and deadlines.
SECTION IV: HUMAN RESOURCES ACKNOWLEDGEMENT - Completed by the Human Resources Office
The Office of Human Resources acknowledges the receipt of the form.
SECTION V: DISTANCE LEARNING CONFIRMATION - Completed by the Director of Cairn Online
The Director of Cairn Online confirms the receipt of the request and will start working with the faculty on the next steps for the course development.
