Remuneration Form

This form is used to review the development of online courses and process the remuneration for the course developer. The form is initiated by the faculty developing a course or the Office of Distance Learning (ODL). Faculty members should refer to the Distance Learning Handbook (Course Development Procedures section) prior to submitting this request.

SECTION I: COURSE  - Completed by the Faculty Member Developing the Course or ODL.

       

 

Course Development Type:   NOTE: Please select the development type that matches the original agreement in the approved Course Development Request Form. The incorrect selection may result in having to redo the form.
   
   


The course development is now complete, and I am requesting a final review by the Office of Distance Learning and the Dean.

       

 

SECTION II: OFFICE OF DISTANCE LEARNING REVIEW AND SIGN-OFF - Completed by the Director of Distance Learning

The Office of Distance Learning has reviewed the course development. The course meets the University requirements to offer it online. 

      

 

SECTION III: DEAN REVIEW AND APPROVAL - Completed by the Dean or the Program Chair

Based on my review, the course meets the curriculum and program requirements. I am requesting remuneration for the faculty course developer as outlined in the Course Development Request Form. 

      

 

SECTION IV:  PROVOST APPROVAL - Completed by the Provost

The Provost approves the remuneration for the course developer as outlined in the Course Development Request Form.

      

 

SECTION V: HUMAN RESOURCES REMUNERATION PROCESSING - Completed by the Human Resources Office

The Office of Human Resources has processed the renumeration for the course developer as outlined in the Course Development Request Form.
